2.  Valid Passport and 1 copy
3. National ID card and 1copy
4. House Registration and 1 copy
5. ID photo 4.5 x 3.5 cm (2 photos)

4. Photocopy of Passport pages that have entry and exit stamps to/from Thailand
5. ID photo 4.5 x 3.5 cm (2 photos)

Fee     3,000 yen per one document
